To view survey responses, use the Activity tab to find the Responses page. This is a live feed of your contact's responses to your survey(s). To view a specific response, click on the report icon. This will open that contact's feedback.

Depending on that specific survey's questions, the tabs on this page will vary. Click thru them on the top right side to see the various question results. 

Note: if a self-assessment still needs to be completed, you may not be able to see this response information yet.
